Healthwatch Leicester asked Young Advisers Leicester (YAL) to consult young people on their views and experiences of transitioning from child to adult health services in Leicester.
Their findings show gaps in understanding, support and services for young people who are moving from Child to Adult Services. Young Advisers make 4 key recommendations around clarity, communication and involving and including young people in their transition planning.
The full report is attached below and was presented to Healthwatch Leicester Board by Jaimini (Young Adviser), who was accompanied by Raakhee (Leicester City Council) on 25th October 2017.
