Consultation with patients about the proposed closure of the 612 Saffron Lane premises and re-location of all services to extended premises at 509 Saffron Lane from April 2019
By increasing the capacity and improving the facilities at our 509 Saffron Lane premises, we will be able to offer a full range of services for all our patients from under one roof and this is why we are proposing to close the 612 Saffron Lane premises once the extension is completed.
Households have been sent a letter, a copy of which is provided your reference and which can be provided in different formats on request if required. Patients have been asked to complete a questionnaire giving their views and feedback and in addition we will also be holding public consultation events at the practice.

There is a Frequently Asked Questions sheet, to help explain the proposed changes. You can look at this either on the practice website www.saffronhealth.co.uk or pick up a copy from our waiting rooms at either 509 or 612 Saffron Lane.
There will also be poster displays in the waiting rooms at both premises as well as information available on the practice website along with a FAQ document for patients to access. The Patient Participation Group will provide additional support by attending the open events and helping to answer queries or concerns from patients.
Your feedback is important and will be used to help us make the right decisions and to ensure we provide a service that meets our patients’ needs.
There are several ways you can have your say. You can pick up a questionnaire from the practice or download a copy from the practice website. The questionnaire is available in regular and large font. You can also fill in our online survey via the survey monkey link https://www.surveymonkey.co.uk/r/patientconsultation2017
The closing date for feedback will be 6th Oct 2017 All feedback we receive from the survey will be taken into consideration before a decision is made, by NHS England in collaboration with Leicester City Clinical Commissioning Group. You will be notified of the decision by letter. We look forward to hearing your comments.
