We would like to hear patient experience feedback and stories of Outpatient and Inpatient Urology services provided by University Hospitals of Leicester NHS Trust, either at one of their hospitals or in the community with a short survey.
Urology Services deal with conditions such as diseases of the kidneys, bladder, urinary tract, male reproductive organs and includes blood-in-urine and prostrate assessment clinics.
This survey work forms part of a wider initiative involving Healthwatch Leicester and Leicester City Clinical Commissioning Group (CCG) focusing on a number of services provided by the University Hospitals of Leicester NHS Trus (UHL). Your story and survey feedback will assist in identifying how those services could be improved and better targeted to meet patient needs.
Any information provided will be treated in the strictest confidence. It will be collated with information submitted by others, anonymised and no personal information will be shared. The collated information will be used by Healthwatch and the Leicester City CCG in their joint reports and presentations.
If you have used these services, please click here to take the survey. Alternatively please circulate this message to your service users and throughout your networks if possible.
If you would like to receive this survey in a paper-copy format please contact Manjit Singh, Patient Experience Manager, at Leicester City CCG on 0116 295 1116. Please note that this survey will close at 8.00am on Monday 13th February 2017.
Many thanks for your help.
